Our services have been re-designed to meet the unique and changing needs of our patients.

Our teams of highly specialized nurses, physicians, physiotherapists, occupational therapists, physical and occupational therapy assistants, social workers, speech and language pathologists, dieticians, pharmacists and recreational therapist work together to provide customized treatment designed to meet the individual care needs of our patients. The program is rooted in a philosophy of care that involves both the patient and family members in the care planning in order to best meet identified needs.

Complex Continuing Care Inpatient Services include:

• Geriatric Assessment and Treatment Unit (GATU): comprehensive geriatric assessment and intervention by interdisciplinary team members to assist frail elderly patients who have sustained significant illness or injury;
• Complex General: complex care for patients with chronic conditions or multi-system disease processes such as diabetes, cardiovascular conditions and stroke;
• Functional Enhancement: low intensity, reactivation program to assist patients who have experienced a recent loss of function due to illness or injury, including stroke and chronic deteriorating illness;
• Alternate Level of Care (ALC): maintenance care for patients who have finished the acute-care phase of treatment and are awaiting placement in Long Term Care Facilities.

Rehabilitation Services include:

• Short-Term Rehabilitation: high intensity general rehabilitation services for inpatients following injury or illness such as hip fractures, joint replacements, stroke and weakness/frailty resulting from surgical and medical conditions;
• Outpatient Rehabilitation

  · Hand Program
  · Musculoskeletal Program (hip and knee replacements, amputations, complex fractures)
