The Geriatric Assessment and Intervention Network (GAIN) Clinic provides comprehensive assessments for frail seniors in the area. It is a collaboration between Rouge Valley Health System and the Central East LHIN, committed to the expansion of specialized geriatric services.

The GAIN Clinic accepts referrals for people generally aged 75 and older who:

• Live at home or in a retirement residence, or

• Have multiple complex medical, functional and psychosocial problems, or

• Are at risk for falls or have experienced falls, or

• Have recently experienced a functional or cognitive decline

The clinic does not accept referrals for seniors residing in long-term care facilities.

The GAIN Clinic health care team

The GAIN Clinic interprofessional team consists of:

• Nurse practitioners

• Physiotherapist

• Occupational therapist

• Social worker

• Pharmacist

• Health career case managers

• Geriatrician or physician who specializes in geriatrics available for consultation when required

Referrals

The GAIN Clinic services must be accessed through a referral by your family doctor or nurse practitioner.

If you are referred for an assessment the clinic may need to contact family members with consent for more information.

Coming for your appointment

The GAIN Clinic is located on the first floor of the Margaret Birch Wing at Rouge Valley Centenary hospital campus.

For your appointment, it is important to come prepared by bringing the following items:

• Walking aids

• Eyeglasses

• Hearing aids,

• All medications, including over-the-counter products (please bring all medication containers, bottles, eye drops, cream, etc.)

• A light lunch/snack, as your assessment may take up to two hours.

Following the clinic visit, a summary of recommendations will be sent to your referring primary care provider or specialist.
