The Emergency Services Program is currently recruiting for 1 part-time Nurse Practitioner (NP-Adult/Primary) with a focus on Geriatric Care within the GAIN clinic. The incumbents will report to the Director –Medicine and Emergency Services, and will work closely with the Chief of Internal Medicine, the Chief Nursing Executive and the Managers and staff within the Program. Working as a member of the inter-professional team, the Nurse Practitioner (NP) will provide specialized comprehensive geriatric services in the Urgent/Emergent GAIN (Geriatric Assessment and Intervention Network) Geriatric Clinic, an exciting new model of Geriatric Outpatient Care. The NP, working with a physician partner or Geriatrician, is responsible for: autonomously diagnosing; ordering and interpreting diagnostic tests; prescribing pharmaceuticals and performing specific procedures within the legislated scope of practice; working in close collaboration with the medical staff to ensure a timely response to a clients changing health status and needs; and providing direct patient care focusing on health promotion and maximizing client’s safety and function to support frail seniors living at home. Actively participating in regular team meetings, liaising with community based providers, advocating on behalf of clients and their families and participation in inter-professional patient and family conferences as needed, this position is ideal for a Nurse Practitioner with a passion for geriatric care. As this new model rolls out, the successful candidate will also participate in program development and evaluation. Hours will require flexibility and will include evening and weekend work. Home visits may be required.

 REQUIREMENTS:

  • Masters degree in Nursing or Health Related Discipline

  • Degree, Diploma or Certificate as a graduate of a recognized Nurse Practitioner Program.

  • Current certification with the College of Nurses of Ontario, holding an extended class license

  • Competent to perform CPR at a Basic Cardiac Life Support level

  • Member of the OHA

  • Eligible for a cross-appointment with an academic institution

  • Minimum 5 years experience in adult nursing

  • Previous Advanced Practice/Administrative/Education experience an asset

  • Demonstrated ability to provide professional practice leadership, to challenge and motivate others and to provide adaptive clinical care sensitive to cultural and ethno-racial diversity

  • Strong research knowledge and skills with the ability to foster same in others, and commitment to enhancing academic endeavours/involvement for self and staff

  • Exposure to Lean principles, an understanding of methodologies and tools and a commitment to the use of these in support of providing the best healthcare experience possible to our patients

  • Evidence of sound organizational and prioritization skills with the ability to work well in stressful situations

  • Excellent communication, negotiation, teaching and interpersonal skills with the ability to work well with patients, families, all levels of staff and the community

  • Ability to work well independently and to exercise sound judgement

  • Ability to perform the essential duties of the position

  • Demonstrated pattern of good attendance and work record
