Requesting your records

You can now request your records of personal health information from Rouge Valley Health System. Please follow these easy steps.

• Step 1: Contact one of our hospital campuses by mail or email, and let our health information management team you would like to get your records:

• Step 2: Download and complete the Consent for Disclosure of Personal Information form, and fax it to the hospital:

Fee structure

A $200.00 pre-paid, non-refundable payment must accompany all third-party requests for the first 10 pages. An extra cost of $1.00 for each additional page will be charged once the cost has been reviewed.

Health information management department

The health information management department at Rouge Valley is responsible for the maintenance, security and release of all personal health information at RVHS. You may come to our office to meet with one of our staff members
