Welcome to online pre-registration at Centenary

At Scarborough and Rouge Hospital, we strive to provide our patients with the best health care experience. We offer online pre-registration for many of our outpatient tests and clinic visits at our Centenary site. Online pre-registration helps to reduce the time that you wait or have to line up.

If you have multiple same day appointments, do not complete the pre-registration form. Patients with multiple appointments occurring on the same day will have their information updated by a registration clerk on the day of the appointment.

How to complete online pre-registration

Required information for pre-registration

For all patients

Additional information needed for elective inpatients who have asked for preferred accommodations (semi-private or private room)

  • Patient's legal name, full address and phone number

  • Patient's Health Card number with version code

  • Name of patient's family physician

  • Date and time of scheduled appointment

  • Name, address and phone number of an emergency contact

  • Patient's room choice (semi private or private)

  • Name of patient’s guarantor (person who holds the insurance)

  • Name of the guarantor’s employer

  • Name of the insurance company that covers the preferred accommodation

  • Insurance policy and identification number

    Please note: When you click on the above link, you will be directed to a separate, secure website called “secure.patientway.com”. This website is operated by PatientWay, an external vendor that is contractually bound to maintain our hospital’s privacy and confidentiality as defined by the Personal Health Information Protection Act, 2004. Neither PatientWay nor Scarborough and Rouge Hospital will use or disclose personal details entered into the pre-registration form for any purposes that do not relate to operating the online pre-registration process.

  • Fill out the form with all of the required information.

  • Click the "Submit Pre-Registration" button at the bottom of the form.

Multiple scheduled appointments

If you have a series of recurring clinic visits, you don't have to pre-register before each appointment as long as 1) you completed the pre-registration form for your first appointment, and 2) your registration information doesn't change throughout the duration of your visits.

 

What to do when you come to the hospital for your appointment

When you arrive at Centenary, you will need to check in. You can do either of the following:

  • Go the admitting desk in the main lobby, and have one of our admitting/registration staff check you in, or

  • Since you are pre-registered, you can do an express check-in at any one of our self-service registration kiosks.

If you choose to use one of our kiosks, check in is fast and simple.

Here's what you will be asked to do:

  • Wash your hands at one of our hand wash stations

  • Swipe or key in your Health Card number (the kiosk will match this to your registration)

  • Verify the registration information that appears on the touch screen

  • Proceed directly to the clinic or department where your appointment is (directions and a map will be provided)

In some instances, your personal information may need to be updated. If this is the case, our kiosks will direct you to the admitting desk where our staff will be happy to update your information.

Not able to pre-register online?

We have tried to make our online pre-registration quick, easy and accessible for all patients. If for some reason you are not able to pre-register online prior to your appointment, please visit the main admitting desk where staff will be happy to help. Please arrive at least 30 minutes before your appointment to allow enough time to complete a full registration. Please ensure that you bring your Health Card with you.
